BEYOND RIDE TRANSPORTATION SERVICE AGREEMENT BEYOND RIDE TRANSPORTATION SERVICE AGREEMENT Beyond Ride Transportation Service AgreementThis Agreement ("Agreement") is entered into by and between Beyond Ride ("Provider") and ________________________________ ("Client"), collectively referred to as "the Parties," for the provision of non-emergency medical transportation and related services.Effective DateScope of ServicesBeyond Ride will provide non-emergency medical transportation services, including ambulatory, wheelchair, extra wide wheelchair, and stretcher transports, as scheduled and requested by Client, in accordance with the terms set forth in this Agreement.Service Standards & Safety CertificationsTo ensure client safety, professionalism, and compliance, every Beyond Ride transport includes: Certified and Vetted Drivers, who are: Drug tested and background checked CPR and First Aid Certified Trained in Bloodborne Pathogen protocols Defensive Driving Certified PASS (Passenger Assistance Safety and Security) Certified Graduates of Beyond Ride’s proprietary certification and safety training Ride Guarantee – All rides booked through Beyond Ride are guaranteed to be fulfilled, barring uncontrollable events such as severe weather or emergencies. Our clients ride with confidence, knowing they are in capable and caring hands.Fare ScheduleServices are billed according to the following standard rates. These rates apply to services provided within the following counties: Whatcom, Skagit, Snohomish, King, Pierce, and Kitsap: Service Type Base Rate Per Mile Ambulatory $25 $6.49 Wheelchair $40 $6.49 Extra Wide Wheelchair $60 $7.99 Stretcher $350 $8.99 Note: Rates may vary slightly by county due to local operational costs. A complete and updated county-by-county breakdown will be made available upon request. Additional Fees: Out-of-Coverage Deadhead Miles: If a pickup or drop-off location is outside Beyond Ride's standard coverage area, a Deadhead Mileage Fee of $2.00 per mile will be charged from the edge of the coverage area to the pickup location. Wait Time: If Beyond Ride drivers are required to wait for clients beyond the included period, a $35 per hour wait time fee will apply. Beyond Ride partners receive one (1) hour of complimentary wait time per ride.Payment TermsInvoices shall be issued on a monthly basis and are due within 30 days of receipt. Any disputes regarding charges must be submitted in writing within 10 days of the invoice date.Rate AdjustmentsBeyond Ride reserves the right to adjust pricing by providing at least 30 days’ written notice to Client through an amendment marked Exhibit A: Fare Change Notice. Such changes will not be retroactive and will apply only to services rendered after the effective date of the amendment.Liability & InsuranceAll rides are insured for $1,000,000 ( One Million Dollars) . Beyond Ride maintains general liability, commercial auto insurance as required by applicable law. Client agrees that Beyond Ride shall not be liable for delays or non-performance resulting from circumstances beyond its control, including but not limited to acts of God, road closures, inclement weather, or medical emergencies. Client agrees to hold Beyond Ride harmless from and against any and all claims, damages, or liabilities arising from services rendered, except in cases of gross negligence or willful misconduct.TerminationEither party may terminate this Agreement with 30 days’ written notice, with or without cause.
